In order to create a MyChart account, we will need some information to match you to your medical record. Please complete all required fields below carefully. If an exact match can't be found, your account will not be linked to your record immediately. If your account doesn't match to your patient record immediately, please call our MyChart Support Line at 864-725-2737 for assistance.

Patients under the age of 12 are not eligible for a MyChart account and must be accessed through a Proxy account. Ask your doctor's office about MyChart proxy access for children under the age of 12. If you have any questions, please contact your doctor’s office.
